In case you procrastinated like I did I went down to jones beach today and there was a big sign saying PERMITS NOT FOR SALE MAIL IN ONLY. So I called Belmont to find out what to do and they said permits can be done via email or snail mail. The first page is instructions.
The second page is the application. The only thing the guy told me was to check the box for what permit I was applying for but don't write anything on the line.
The third page is authorization to run your credit card since you can't email cash or checks.
