I would like to obtain copy of Certificate of Occupancy and Survey of my house.

I emailed Freedom of Information Law (FOIL) and filled out am application. They wrote back:

The Office of General Services has conducted a diligent search but does not possess records responsive to your request. However, Suffolk County may possess the records you seek. Please submit a request to this agency. ​

Does anyone know what agency I should contact? Any help or links appreciated.

I would start with your local towns building inspector office.
